Two canopies out usually happens if the AAD fired during your main deployment (you missed deployment altitude and pulled too low) or if the reserve pin was placed incorrectly. If you can see two canopies out you need to correct it or to steer it. This may result in a dangerous main-reserve entanglement. More commonly, the two canopies will arrange themselves in a Biplane, Side-by-side, or a dangerous Downplane configuration.
